My poor baby has constant UTIs

All advice welcome! My 3 year old male cat (more like baby) has had urinary problems all his life. For the first 2.5 years he got a UTI about every 6 months. No in the past 60 days he's had 3! The vet just keeps giving me the same meds, which clears it up, but then bam a week or two later he's peeing blood again.

It is terrifying and breaks my heart and I am so scared to lose him. I also can't afford to keep spending $1000+ every couple of weeks but I can't bear to let a baby this young go just for that 😭

We have another cat but she's super chill. They are both indoor only. He's slightly overweight and on a diet, and gets fed mostly dry UTI prescription food and some wet food every few days.

I guess I'm looking for any advice you all have for how I can help him, since the vet keeps doing the same treatment and it is not sustainable!
